One thing is clear. Cosmetic surgery stems from low self-esteem and non-acceptance of ourselves. If we truly liked and loved ourselves we wouldn’t feel the need to change anything. Women especially, are conditioned to look good. There is so much pressure to be beautiful and thin and it causes emotional distress and immense amounts of self-loathing in women all over the world.

We are bombarded with perfect images of women on a daily basis. Just look up at an advertising billboard or read a magazine and perfection stares back at you. It is easy to forget that the image you see is probably the best one of a hundred photos taken and has more than likely been airbrushed as well. Yet, these pictures of slim attractive women trigger our insecurities and self-doubt and usually cause us to reach for the first doughnut we can lay our hands on…and so the self-loathing continues.

The cosmetic surgery industry is booming and is likely to continue doing so. Yet it seems there is not enough being done to educate people from the inside out. We automatically assume that beauty on the outside will transform us within but this is wrong. Most women do not adore themselves overnight once surgery has taken place. Many find they are still stuck with the same old insecurities as before and they still compare themselves unfavourably to other women. Surgery can help individuals with deformities and obvious flaws but those women looking for superficial changes – such as breast enlargements, will more than likely find that they don’t miraculously change into the fictional women they always hoped to be. They still live with the same attitudes and beliefs as before.

Plastic Surgery Overview

Plastic surgery has witnessed a growing trend, with increasing numbers of individuals opting for aesthetic procedures to enhance their physical appearance. The appeal often stems from societal influences, including the emphasis on beauty standards in media and social platforms, as well as a desire for improved self-esteem and confidence through perceived physical improvements. While plastic surgery can offer desired physical transformations, it comes with inherent risks and disadvantages. Less-discussed negative outcomes may include: [1]

  • Dissatisfaction with results
  • Emotional distress
  • Unforeseen complications such as infections or scarring
  • Swelling and bruising

This article focuses on the disadvantages of plastic surgery and how to weigh these pros and cons effectively.

What Are the Pros and Cons of Plastic Surgery?

There are numerous pros and cons associated with cosmetic surgery. Pros can include increased self-confidence, improved physical health (for instance, rhinoplasty can help with breathing problems), and even improved mental health by reducing anxiety and depression related to one’s appearance. However, there are also cons to consider. Cosmetic surgical procedures can carry the risks of complications such as infections, scarring, and even life-threatening problems like blood clots or anesthesia complications. There may be dissatisfaction with the results, potentially leading to the need for additional cosmetic surgeries. Furthermore, these procedures can be expensive and may not be covered by insurance.

I have given it some thought and here are the pros and cons of cosmetic plastic surgery procedures from a Psychologist’s viewpoint.

What Are the Disadvantages of Plastic Surgery? 

  • Health risks : Surgical complications and adverse reactions to anesthesia or other medications.
  • Psychological harms : Potential dissatisfaction with results and the risk of developing mental health issues.
  • Economic burden : Involves initial costs of surgery and potential expenses for future corrective procedures.
  • Societal and ethical implications : Contributes to the perpetuation of unattainable beauty ideals and raises ethical considerations regarding body image and societal pressures

The Pros of Plastic Surgery

1) Cosmetic surgery can improve body appearance and benefit a person on a superficial level

2) Confidence can be improved by altering the attitude of the person having the cosmetic and plastic surgery done. Some people need very little to jump start their image of themselves in a positive direction and tweaking their physical appearance with certain procedures can be the boost they need in order to see themselves differently. It is not just about the surgery in itself though, there is a change in attitude that helps the transformation

3) Cosmetic surgery helps people to feel more accepted by others and less inferior to some degree.

The Cons of Plastic Surgery

1) Cosmetic surgery can make someone feel less confident as they will always be thinking that others like a ‘fake version’ of them. This is not the true reflection as nature intended and it can lead to a person feeling even less in tune with themselves. They are likely to wonder whether people would still like them if they knew them pre-surgery.

2) Cosmetic surgery deals solely with aesthetics and does nothing for the emotional and mental aspects of a person. This can leave a person feeling even more lost than before they undergo the surgery.

3) Complications of surgery. There is always a risk of pain when undergoing surgery.

Ultimately, there is no right or wrong here. Cosmetic surgery is an individual choice. Some people swear that it has changed their lives for the better whilst others feel that they had unrealistic expectations of how cosmetic surgery could change their lives. It won’t make people love you anymore than they already do (and if it does then those people are incredibly shallow) and it won’t make you the most popular person at the party. Make sure that you are doing it for the right reasons – to please yourself and improve upon yourself not to fit in with others or fit the ideals of others.

Health Risks and Complications

As with any surgical procedure, cosmetic and plastic surgery do come with some medical risks, some of which are specific to each type of surgery. However, some risks that come with all plastic surgeries include:[2]

  • Hemorrhage (excessive bleeding)
  • Blood clots
  • Fluid accumulation
  • Organ or nerve damage
  • Anesthesia complications like an allergic reaction, cardiac arrest, respiratory failure, and coma

Additionally, another major risk is that of an unsatisfactory outcome, potentially leading to the need for revision surgeries. Factors like unexpected healing patterns, asymmetry, or dissatisfaction may cause people to undergo additional procedures.

One study on aesthetic surgeries found that over 8% of procedures needed revisions. Of those, nearly 14% were for thigh lifts, almost 16% were for rhinoplasties or septoplasties, and over 27% were for otoplasties (ear surgery).[3]

Psychological and Emotional Aftermath

While many people are convinced that plastic or cosmetic surgery will fix their issues or improve their mental health, these procedures can also worsen psychological or emotional health. 

Research suggests that a subset of individuals may experience dissatisfaction with their procedure outcomes. Studies have reported cases of postoperative regret, with factors such as unmet expectations, unrealistic aesthetic goals, and psychological factors contributing to negative post-surgery experiences. [1]

Although cosmetic surgery may help improve a person’s self-esteem or satisfaction with their appearance, oftentimes, it doesn’t fix the problem. Many people who seek out plastic surgery, especially several procedures, may have a condition called body dysmorphic disorder (BDD). This mental health disorder involves an obsessive fixation on a perceived flaw in appearance.[4],[5],[6] 

For example, someone may fixate on the shape of their nose and once they get surgery, they may move on to obsessing over their chin or cheekbones. These individuals are typically dissatisfied with their results because the problem is psychological as opposed to physical. With growing awareness of this disorder, many plastic surgeons will not perform procedures on people with body dysmorphic disorder.[4],[5],[6]

Is Cosmetic Surgery Good or Bad?

The evaluation of cosmetic surgery as “good” or “bad” is highly subjective and depends on various factors including the individual’s motivations, expectations, and the potential risks involved. For some people, cosmetic surgery may lead to improved self-esteem and improved quality of life. For instance, someone who has undergone liposuction might feel more comfortable in their skin and experience reduced anxiety about their appearance. However, like any surgical procedure, cosmetic surgery comes with risks, including complications during surgery or dissatisfaction with the results. Therefore, it’s crucial for individuals who undergo cosmetic surgery to thoroughly research, discuss with medical professionals, and carefully consider their decision before undergoing such procedures.

How Does Plastic Surgery Affect Relationships?

Plastic surgery can affect relationships in a variety of ways, both positive and negative. On the positive side, if an individual feels more confident and comfortable with their appearance after surgery, this can potentially improve their interpersonal relationships. However, changes in appearance may lead to issues within relationships. For instance, a partner or spouse undergoing cosmetic surgery may have difficulty adjusting to the change, or might feel insecure or threatened. Furthermore, if the surgery was done to meet unrealistic expectations or without the full support of a partner, it could potentially strain the relationship.

Why Are People Against Plastic Surgery?

There can be multiple reasons why people are against plastic surgery. Some individuals may hold a philosophical belief that one should accept and value their natural appearance rather than altering it surgically. Others may have concerns about the risks associated with a plastic surgery procedure, the potential for complications, or dissatisfaction with the results. There is also a societal debate around the pressures placed particularly on women to conform to certain beauty standards, which can be perpetuated by the availability and normalization of cosmetic procedures. These individuals may argue for a focus on boosting self-confidence and promoting beauty in all forms, rather than resorting to surgical alteration.

Because cosmetic surgery can bring lasting and dramatic changes to your outside appearance, it is important to understand how these changes might affect you on the inside. Before making an appointment to see a cosmetic surgeon, consider your motives for wanting to change how you look.

Many physical characteristics can be successfully changed through cosmetic surgery; others cannot. Good candidates for cosmetic surgery:

  • Have realistic expectations about what can be accomplished
  • Understand the medical risks, physical effects during healing, how the surgery will affect them personally and professionally, what lifestyle changes may accompany the recovery period, and the expenses involved
  • Have chronic medical conditions under control
  • Have no history of smoking or commit to abstain from smoking and nicotine products, including chewing tobacco and nicotine patches, gums, or lozenges for four to six weeks before and after surgery
  • Have had a stable weight for six to 12 months, for certain procedures

All surgeries, including cosmetic procedures, carry risk. If your body mass index is 30 or higher (obesity) or you have diabetes, you might be at higher risk of developing complications such as blood clots in the legs or lungs. Smoking also increases risks and interferes with healing.

You'll meet with your surgeon to discuss these risks and others related to your health history.

Possible complications for any surgical procedure include:

  • Complications related to anesthesia, including pneumonia, blood clots and, rarely, death
  • Infection at the incision site, which may worsen scarring and require additional surgery

Before the procedure

Your surgeon will explain how cosmetic surgery can change your body and what you can expect as a result. This is an opportunity for you to explain what you hope to achieve with surgery. Understanding the options and possible outcomes will help you make the best decision.

You will be informed about specific procedures, what to expect, the benefits, risks and possible complications, as well as other alternatives. The doctor might recommend additional procedures to enhance your overall result.

The concept of asymmetry will be explained. The human body is asymmetric, meaning one side of the body looks naturally different from the other. Consider the image reflected in the mirror — a photographic image is a true image, the opposite of the mirror image. Introduction.

Currently, cosmetic surgery is one of the most popular ways to look more attractive and younger. Women all over the world use cosmetic surgery for numerous reasons. Some of them dream to attract men’s attention, some just want to perfect their appearance, some women wish to stay young as long as possible. Cosmetic surgery gained such wide popularity about five years ago. Some women have already used this service, some still dream to do it regardless of the age, origin and social status. There are numerous reasons to go under the plastic surgeon’s knife. Women’s desire to stay young and beautiful forever makes them forget about certain risks of the cosmetic operations. Modern society influences the women’s decision to change themselves significantly. It dictates the standards of female beauty through mass media day by day (Harris-Moore 139). Thus, society is responsible for the outcome of the operation to some extent. The degree of public influence should be analyzed in order to assess the advantages and disadvantages of cosmetic surgery. Besides the obvious advantages of this service, there are a number of considerable disadvantages.

In addition to the risks connected with physical health, cosmetic surgery may cause substantial harm to the psychological state of the woman. In addition, plastic surgery is a fairly expensive service, and not everyone can afford it. Often the price depends on the professional skills of the surgeon and the quality of the material he uses. There are a great number of tragic examples of failed plastic surgery and its terrible consequences. There are cases of suicides provoked by bad plastic surgery. Furthermore, there are various problems associated with a patient’s age. Young people can easier survive the consequences of the plastic surgery. Nevertheless, the disadvantages of the cosmetic surgery are evident. The person experiences not only physical but also moral discomfort. The society which made the woman do the surgery judges her for surgical misadventure. People make sacrifices for plastic operations, they lend money, take loans, and there are even cases of theft. Unquestionably, people have the right to do what they want but if their actions do not harm other people. Modern society imposes the cult of physical beauty. Trying to achieve the ideal appearance, people forgot about their spiritual development. Their every action is just a tribute to fashion. Unfortunately, it is fashionable to be slim and attractive, but not to be kind and sincere. Our appearance is given to us by nature and parental genes, it is unique and natural. People should appreciate it.

Every day rich and famous look at us from the screens. They constantly tell about their successful plastic surgeries. Women dream to look like their idols, to be slim and beautiful forever. In recent years, cases with fatal outcome of the operation became more frequent. In spite of this, cosmetic surgery becomes more and more popular. Today it is the usual procedure; people do not treat it as something dangerous and risky. Not everything depends on the money you pay for it or surgeon you choose. People should realize that it is a serious surgery, and something may go wrong. Nothing insures people against the failed operation (Kleeman 29). People always have the choice to do it or not. First of all, they should be responsible for their health if they do not want to die young and beautiful. Very often public opinion influences men also. They persuade women to change the appearance. Constant pressure from the beloved person makes women venture upon this step (Kleeman 32). Another significant drawback of the cosmetic surgery and its popularity is that fact that teenagers inspired by the beauty of the celebrities want to look like them. About a half of girls aged between 7 and 16 dream to change something in their appearance with the help of cosmetic surgery. This industry should be regulated strictly by the state (Martinson). Of course, the mass media also should be stopped inspiring people, especially morally immature, to use the services of the plastic surgery. There are a number of different medical supplies available on the Internet. Everyone may buy it and use improperly. Thus, the danger is not even in the access to these medicines, it is in propaganda and advertising of the artificial beauty (Martinson). The increasing demand for this procedure makes this industry and its influence upon people almost uncontrolled. These medical services are not aimed at saving a person’s health; their primary goal is to make money. They do not provide any guarantees to their clients. Every operation is a matter of luck. There are many cases of litigations with the clinics because of the failed plastic surgery. They are more like the business corporations than the medical institutions. The patient is like a business partner, who enters into a contract without any guarantees. It makes the client of the clinic totally defenseless in case of surgeon’s failure.

One more indisputable drawback of this procedure and of this industry at all is that it popularizes the idea that plastic surgery is the cure-all for every problem. Often women think that the operation will save them from divorce, bankruptcy, old age, poverty, dismissal, etc. However, after surgery, they understand that nothing changes, and except these troubles they have serious problems with their health. Subsequently, protracted depression and despair expect them. In addition, relatives and friends also suffer, because they cannot help her.

Another significant disadvantage of plastic surgery is that even successful result is not final. Women want to look perfect without any efforts. For example, after liposuction women start to eat everything they want and gain the weight again. They continue to do this operation over and over again, hoping for long-lasting results. They do not even realize how insidious is the effect of body modifications on human health (Davis 19). Very often failed surgery divides woman’s life into before and after. Great hopes are replaced by a great disappointment. She realizes that she had a multitude of supporters before the operation, but all of them are indifferent to her personal problems afterward. She is left alone with her psychological problems. In most cases, lonely and diffident women are more likely to do the cosmetic surgery than happy, married women. Thus, not only popular media are the factors influencing women’s choice.

The disadvantages of the cosmetic surgery are incontestable. Every year, hundreds of women suffer the consequences of the failed cosmetic surgery. Today it is one of the most urgent problems of healthcare that should be solved. Popular mass media are the efficient mean of the influence on women’s attitude to the plastic surgery. The cult of health should replace the cult of beauty. Natural beauty and sound health are priceless.

The American society for Aesthetic Plastic Surgery (ASAPS) reports that in 2003, approximately 7.3 million women underwent cosmetic procedures while an estimated 1.1million man had cosmetic procedures. Liposuction was the common procedure for both women and men with breast augmentation coming in second for women.

The increase in people having cosmetic surgery points to a reduced concern for the dangers associated with plastic surgery. It has also been noted that there is an increase in the number of women younger than 18 years who are having cosmetic surgery. While the actual numbers indicate that those teenagers below 18 undergoing cosmetic surgery form a small percentage of the total, the worrying trend is the 48% increase noted in their numbers from 2002 to 2003.

Cosmetic surgery for a teenager is not only dangerous because of medical risks involved there is also the additional aspect that the way the young patient’s body is changing will be affected. Sometimes a small increase in weight results in changes in the size of the breast. Due to the fact that the patient is still growing, altering her body parts may lead to dissatisfaction later in life when the patient is fully mature.

Complications of cosmetic surgery range from minor problems such as scarring to more serious complications like fatalities. Many cosmetic surgeons attempt to hide incision lines by putting them in places where they are hardly noticeable. For example for a breast augmentation, this could be along the crease of the breast while for facial surgery; the incision line is hidden in the hairline. Permanent scarring is a common result of most surgeries. There is a probability of bleeding and infection with every surgery but these are complications that can be managed without posing a serious health risk (Washington, 2006, Pg 2)

Nerve damage is one of the more serious risks associated with plastic surgery. This may manifest with loss of feeling in the operated areas and difficulty in muscle movement. Generally the risk of a cosmetic surgery procedure differs depending on the specific procedure. For example breast augmentation will have such risks as nerve damage at the site of operation, tissue necrosis, infection, scar tissue formation and asymmetry. Premature aging, features that look abnormal and visible tissue damage are some of the risks associated with facial plastic surgery (Washington, 2006 Pg 2).

It is necessary to have tests done and undergo a physical examination done before going through with a cosmetic surgery because then certain conditions that would put an individual at risk can be discovered. Clients with diabetes, cardiovascular conditions and allergic conditions are at a greater risk of developing complications that other people would normally not get.

Many times patients report that the results of the surgery are usually different from what they had anticipated. A story in the Telegraph illustrates this; a mother of four went to a hospital for a liposuction but the whole operation got botched and she sustained puncture wounds to her colon and bowel, which led to blood poisoning and multi organ failure causing her to have cardiac arrest for four minutes (Bunyan, 2006, Pp 1).

Another illustration of how cosmetic surgery can go wrong is provided by ITV’s documentary, ‘Facelift from Hell’ which follows a group of ladies who underwent eye, facial and stomach surgery in Poland. The documentary provides insight into the degree of agony the women underwent, one of the women even claimed that following the surgery she had not been able to open her mouth properly for a period of 18 months. (Bunyan, 2006).

An FDA study indicates that for women who have had silicone implants for a period of about 6-10 years, approximately 50% will have a broken implant and 21%of them will experience migration of implant to another body part. There are possibilities of raptures and leakages together with complications like fibromyalgia and thyroid disease (Canadian Women’s Health Network, 2003).

Silicone implants have gradually been replaced with saline filled ones. A study by the Mayo Clinic resulted in evidence that 25% of women who had breast implants required more surgery within a span of five years (Tweed, 2003). Adverse reactions are not only associated with silicone implants but with saline implants too.

The complications with breast implants include surgical complications systemic and psychological complications. The local complications are those likely to be experienced by all patients who go for surgery. These include complications related to being anaesthetized, infection, delayed healing of wounds and increased possibility of other surgeries. Some of the local complications include formation of contractures in the capsule which occurs on average in within the first two years of surgery for those women who have implants (Tweed, 2003). Contracture formation is very uncomfortable and excessively painful.

The implant usually deteriorates with time. A US government study indicates that in 344 women with implants 2/3 rd of them had ruptured implants upon MRI examination (Tweed, 2003 pp1). Deflation and rupture results in the filling of the implant being spread throughout the body. Chest discomfort and pain, discharge from the nipple and interference with the mammography are other possible local complications (Tweed, 2003 Pp1)

Systemic complications usually occur several years following implantation. Though the link between systemic complications and breast implantations is unclear, research from epidemiology indicates that implants are a vulnerability factor. Often, the complications appear as a group of symptoms associated with conditions like autoimmune diseases, fibrositis disorders and connective tissue conditions. Involvement of lymph nodes and granuloma formation are other conditions reported by women who have implants (Tweed, 2003, Pp1).

Many surgeons report that most patients are happy and satisfied with breast augmentation. This may be so for a while especially the immediate post-operative period. Many of the reports of satisfaction are given by the cosmetic surgeons themselves raising questions about the credibility of the reports. This is because it is highly unlikely that a patient will admit to the cosmetic surgeon (face-to-face) that they are dissatisfied. Most surgeons will also not admit to other surgeons that their patients are dissatisfied.

There are studies that indicate that women who have not undergone breast augmentation have lower levels of breast anxiety, dissatisfaction with their upper torso and depression compared to those who undergo implantation. Those who undergo implantation and ‘explantation’ have high levels of anxiety both before and after ‘explantation’ (Tweed, 2003, Pp2). Consequently, the most viable conclusion is that contrary to popular belief breast augmentation has negative psychological effects.

Liposuction the most common cosmetic surgical procedure also has dangers associated with it. The most common complications are scars, and irregularities of contours following liposuction. These irregularities increase the need for surgery due to the loose skin that is commonly left hanging especially for older patients and those who already have stretch marks. The more significant and distressing complications of liposuction include swelling, infection, damage of organs, pulmonary embolism and fluid imbalance among others.

Edema (swelling) may occur for several months after liposuction. This has the effect of numbing nerves and the patient may have a tingling effect. Organ damage is a more severe complication which may occur due to the fact that the surgeon probes using a cannula inserted in a small incision. The intestines may be punctured because visibility is not adequate. For a patient with a hernia (intestine protruding out of the abdominal wall) the risk of internal organ damage is increased.

Liposuction may lead to dislodging of small pieces of fat which may travel to the lungs. Once here, they cause pulmonary emboli. This situation compromises breathing and is an emergency situation that if not attended to immediately could pose a fatal health risk. Most patients have the highest vulnerability to pulmonary embolism, in the first 3 days post-operation. Fluid imbalance is a complication that arises from the use of tumescent liposuction a procedure whereby fluids are injected in large amounts and the fat tissue also has fluid removed from it. The potential for fluid imbalance is high and when the imbalance is not treated pulmonary edema results. Heart and kidney dysfunction may also result.

Though the actual deaths associated with liposuction are on average 3 for every 100, 000, the effects of liposuction cannot and should not be ignored as these complications do not have physical effects but also have psychological implications and monetary implications which work together to influence the health of the patient negatively and sometimes contribute to death.

Facelifts also have their own complications and in some cases have been known to cause deaths primarily because of the need to anaesthetize the patient. With cosmetic surgeries usually, one is never enough and continually going under the knife increases the threshold for the anesthesia to be effective. Afsun Smith reports that a media mogul in the UK Micheline Charest died after undergoing a facelift (Smith, 2007, Pg1)

The major problem associated with the dangers of plastic surgery is that most surgeons do not tell their patients of the risks associated with the surgeries. The patients therefore are not in a position to make informed decisions. The fact that most of the surgeries are done in private clinics also contributes to the complications and fatalities associated with plastic surgery. When a patient gets an emergency complication, rushing them from the private clinic to the emergency room in another hospital lowers the survival chances of the patients with a complication.

To cut costs of operation plastic surgeons sometimes work without anesthetists. This is a situation that puts the client at risk. In Florida the Health Department withdrew the license of one doctor whose failure to work with an anesthetist led to a patient’s death. The doctor did not respond in time when the patient had difficulty in breathing leading to cardiac arrest and finally the death of the patient (Smith, 2007, Pg 1).

The seriousness of the danger associated with plastic surgery is compounded by the fact that some Americans are willing to undergo cosmetic surgery where the cost is cheap without consideration for the risks involved. Victoria Corderi of the Dateline NBC reports on the dangers of having plastic surgery in countries where it is cheap like the Dominican Republic. She gives an account of different women who came back with infections so severe they would have caused their deaths. Some women even died as a result of the liposuctions while others came back with infections whose causative organisms are unknown causing the New York Health Department to put out an alert (Corderi, 2005, pp1-3).

Despite the fact that technology has decreased the risks associated with cosmetic surgery more awareness needs to be created to ensure patients can make informed decisions or even better overcome the desire to go under the knife when they realize how dangerous the cosmetic procedure can become. In addition, close supervision of cosmetic surgery clinics for quality care is necessary if the dangers are to be decreased.
